M.A.P. Privacy Policy

My Autism Passport (M.A.P.) Privacy Policy

Autism Films Inc. (“Company”) and St. Michael’s Hospital (“SMH”) are concerned about safeguarding your privacy.  Company’s Privacy Policy is based upon the principles set out in the Canadian Personal Protection and Electronic Documents Act (PIPEDA) and complies with PIPEDA and all other applicable Federal and Provincial (Manitoba) legislation.

This Privacy Policy applies to the “My Autism Passport” mobile application (the “Mobile App”) except if you have elected to participate in the SMH Research Study to measure the effectiveness of the Mobile App this Privacy Policy does not apply to private information collected by SMH (see below).

The Mobile App is intended as a tool to be used by parents/guardians of children with autism spectrum disorder to facilitate accessing regional developmental and medical services, tracking the care received by their child, and providing a concise, at-a-glance summary of the therapy and interventions their child has received.  As such, the more information you enter into the Mobile App, the more effective it will be.

On first use of the Mobile App you will be asked for your location/address, your child’s age, gender, whether your child has been diagnosed and the date or diagnosis, and details of the diagnosis and additional diagnoses. Unless you have elected to participate in the SMH Research Study, all personally identifiable information and personal medical information input into the Mobile App resides solely on your mobile device and is not uploaded, accessed by Company or SMH, or shared with anyone in any way, unless and until you decide to disclose/share this information.

The Mobile App offers you the opportunity of sharing the notes that you’ve entered into the Mobile App by means of text messaging, email, printing, among others. In this way you alone control the disclosure of these notes.

In summary, except if you have elected to participate in the SMH Research Study, neither the Company nor SMH collects any personally identifiable information or personal medical information (collectively referred to below as “Private Information”), all your input into the Mobile App resides solely on your mobile device.

If you elect to participate in the SMH Research Study SMH will provide you with a separate Consent Form which will describe the purpose, methodology, and benefits of the Research Study.  As well, the Consent Form will inform you of the nature and purpose of the Private Information collected for the purposes of the Research Study, and the steps taken by SMH to safeguard that Private Information in accordance with applicable privacy legislation including the Personal Health Information Protection Act of Ontario. If you participate in the SMH Research Study, the Consent Form, and not this Privacy Policy, applies to your Private Information collected by SMH in connection with the Research Study. None of this Private Information is shared with Company.

Even though neither Company nor SMH collects or accesses your Private Information (unless you are part of the Research Study), Company has established the following Privacy Policy in the event that Private Information is collected in the future.

Company’s Privacy Policy is based upon the following ten basic principles:

Principle 1:  Accountability

The Mobile App provides links to other third party websites.  These websites operate independently from the Mobile App and therefore any personally identifiable information (or Private Information) you give to these other websites is not controlled by Company or this Privacy Policy.

Similarly, if you elect to share your notes through the sharing functions of the Mobile App such disclosure is not controlled by Company or governed by this Privacy Policy.
