The Mobile App is intended as a tool to be used by parents/guardians of children with autism spectrum disorder to facilitate accessing regional developmental and medical services, tracking the care received by their child, and providing a concise, at-a-glance summary of the therapy and interventions their child has received. As such, the more information you enter into the Mobile App, the more effective it will be.
On first use of the Mobile App you will be asked for your location/address, your child’s age, gender, whether your child has been diagnosed and the date or diagnosis, and details of the diagnosis and additional diagnoses. Unless you have elected to participate in the SMH Research Study, all personally identifiable information and personal medical information input into the Mobile App resides solely on your mobile device and is not uploaded, accessed by Company or SMH, or shared with anyone in any way, unless and until you decide to disclose/share this information.
The Mobile App offers you the opportunity of sharing the notes that you’ve entered into the Mobile App by means of text messaging, email, printing, among others. In this way you alone control the disclosure of these notes.
In summary, except if you have elected to participate in the SMH Research Study, neither the Company nor SMH collects any personally identifiable information or personal medical information (collectively referred to below as “Private Information”), all your input into the Mobile App resides solely on your mobile device.
Principle 1: Accountability
Personally identifiable information may include your or your child’s name, age, address, email address and photo.
Principle 2: Identifying Purpose
If in the future Company wishes to collect your personally identifiable information, the purposes for which personally identifiable information is collected shall be identified on the Mobile App at or before the time the information is collected. Personally identifiable information collected from you for one purpose will not be used for any other purpose without your prior consent.
We do not sell or otherwise share personally identifiable information without your prior approval; if applicable we will ask for your approval at the place on the Mobile App where we collect the information. We will not send you materials unless you expressly “opt in” to receive such materials.

The collection of personally identifiable information shall be limited to that which is necessary for the purpose identified in the Mobile App at the time of collection. Information shall be collected by fair and lawful means.
Certain non-personally identifiable information may be recorded by our web-servers and hosts, and will provide aggregate statistics such as type of device (Android or iOS) being used, average session length, number of users accessing Services Providers, average distance to Services Providers being accessed, etc. but no device ID, device IP address, or personal information will be collected.
Principle 5: Limiting Use, Disclosure And Information
Personally identifiable information shall not be used or disclosed for purposes other than those for which it was collected, except with your prior consent or as required by law. Personally identifiable information shall be retained only as long as necessary for the fulfillment of those purposes. Company may retain your personally identifiable information on our mailing lists for the purpose of providing information to you, until you request to be unsubscribed from such mailing lists.
Principle 6: Accuracy
Personally identifiable information shall be as accurate, complete, and up-to-date as is necessary for the purposes for which it is to be used. Company will periodically update its records to keep the information current.
Principle 7: Safeguards
Personally identifiable information shall be protected by security safeguards appropriate to the sensitivity of the information.
Principle 8: Openness
Company shall make readily available to you specific information about our policies and practices relating to the management of personally identifiable information.
Principle 9: Individual Access
Upon request, an individual shall be informed of the existence, use, and disclosure of his or her personally identifiable information and shall be given access to that information. You shall be able to challenge the accuracy and completeness of the information and have it amended as appropriate.
